CONDITIONS OF SERVICE
Medical Information
You will be asked to provide us with information related to your health, which may be relevant in the course of any treatment we have arranged on your behalf. We will send you a medical questionnaire by e-mail. In providing the information, you guarantee that it is accurate, correct and current. For surgery and dentistry you will be required to provide us with clear photographs of the area being treated:

  • For surgical procedures this will be front and sides, as it is important for the surgeon to be able to examine skin quality and overall shaping of the contours.
  • A panoramic X-ray of your teeth may be required for implants.
  • A blood test may be required.


You agree that you will send us details of your medical history, along with any requested photographs and information, via e-mail. We will ensure that any information given to us is treated in a confidential manner in accordance with our Privacy Policy.
Quotations
A breakdown of costs will be included in your quotation.
The quotation we provide will be confirmed at your pre-treatment consultation, prior to any procedure taking place.
The quotation includes only the services detailed in it. If any non-predicted costs develop once you are in Poland, they will be detailed and agree by you in order to obtain your authorisation to proceed.
In the unlikely case of unforeseen complications the quotation does not include any additional medical expenses for medication or procedures. Please ensure that you discuss this with the surgeon before the operation.
Deposit
We require a deposit of 650 PLN (about £145) payable at the time of booking services. This is an administration fee, it forms part of the overall cost of your procedure and will be deducted from your final invoice. This amount is not refundable should you decide to cancel your booking. However, should you decide to book another service within 12 months you may transfer the amount paid, and it will be deducted from your subsequent booking.

Price Changes
Before your booking is confirmed and a contract comes into existence we reserve the right to increase or decrease brochure prices.
Liability
ClinicForYou.com takes responsibility for arranging services and support in Poland. Your doctor/surgeon takes responsibility for your medical treatment.
